
I’m Dr. Dalian, a board-certified family physician who believes that health goes far beyond prescriptions and lab results. It is shaped by how we live, think, eat, move, rest, and navigate the different seasons of life.
I was born and raised in Puerto Rico and moved to the United States in 2009 to continue my medical training. In 2017, I founded Your Family MD using a direct primary care model. Working directly with patients rather than insurance companies gave me the time to have deeper conversations, build meaningful relationships, and provide care focused on the whole person, not only a diagnosis.
As my practice evolved, I began to see that many patients needed more than traditional primary care alone. They needed support that considered metabolic health, hormones, brain health, nutrition, lifestyle, stress, sleep, and mindset alongside medical treatment. This led me to expand my education in functional medicine, weight and metabolic health, nutrition, brain health, perimenopause and menopause care, and health and life coaching.
In 2025, I became an Elite Brain Health Certified Clinician through Amen University, completed advanced training in perimenopause and menopause hormone therapy through the Heather Hirsch Academy, and began formal training with the Institute for Functional Medicine. I continue my education in functional medicine, obesity medicine, and culinary medicine so I can provide thoughtful, current, and evidence-based care.
What began as a full-scope traditional primary care practice has grown into something more focused and intentional. I rebranded my practice as Dalian MD to better reflect my commitment to personalized, holistic care. Today, my work centers on metabolic health, chronic disease prevention, hormones, brain health, and sustainable lifestyle change.
My approach is not about quick fixes. It is about identifying patterns and underlying factors that may be affecting your health, helping you understand what is happening in your body, and creating practical strategies you can carry into everyday life.
I provide care primarily through telemedicine, offering greater flexibility and accessibility. I may also offer limited in-person visits when appropriate. This allows me to provide care that fits each patient’s needs, priorities, and goals.
My focus is not only on treating illness. I want to help people build lasting health so they can feel better, think more clearly, and fully enjoy their lives.
Outside of medicine, my faith and family are at the center of my life. I serve as a volunteer leader for middle and high school students at my church, which has become one of the greatest joys and privileges of my life. I am passionate about helping people grow and reach their potential, whether through healthcare, mentorship, or education.
I also enjoy reading, gardening, watching sunsets, discovering new coffee shops, and having meaningful conversations about health, faith, personal growth, and life. These simple moments help keep me grounded and remind me that health is about much more than what happens inside a doctor’s office.
